Register with us as a New Patient

How do I Register?

All patients living in our practice area can register to be a patient. We ask that you complete a health questionnaire and NHS England registration form (GMS1). Both these forms can be found below and brought to the surgery filled out and signed. Could all registering patients please bring proof of ID and address.

We will also provide patients with log on details to book appointments and order medications online as standard, but if you would also like access to your medical records online we ask that you complete the online access form as well.

You can now also register with the practice online . You will need to complete the full registration as directed and may need to be seen by the practice nurse to fully complete the registration process so please ensure the correct contact details are entered. Please ensure you are living in the practice area before completing the registration process.

Register at our GP Practice

Practice Area

Our practice area covers most of central Gateshead, but also extends to Lobley Hill, Whickham, Dunston, and Low Fell. We welcome all patients from these areas, please ask at reception for more details.

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

We have the right to refuse to register patients if they have a violent flag against their record.

Download a form to opt out of having a Summary Care Record (SCR) or opt out of additional information being included in the SCR.

Organ Donation

From 2020, adults in England will be considered potential donors unless they choose to opt out or are excluded. For more information please visit the Organ Donation NHS UK where you can also register your decision.
